Patient Forms

To help you save time, we have provided patient forms online so you can fill them out before your visit! If you have questions, then feel free to call our office during regular business hours to speak with one of our staff.

It is our intent to always provide your child the best care possible and to accommodate as many requests as is realistic and feasible.  It is within this context that we ask that you take a few moments to review policies that affect the way services are provided at Shining Star Pediatrics.

Arrive Early. Please remember that all insurance companies require that your data be updated prior to each encounter. This usually takes several minutes. If this is not done, your insurance may deny your claim. We do not want time spent on administration requirements to limit your time with the doctor. Schedule an appointment by calling 903-212-6060. You may schedule same day appointments for ill visits on a first available appointment basis.

Late Arrivals And No Shows.  Patients who have arrived on time will be seen ahead of those who arrive late.  If you arrive late, we may need to reschedule your child’s visit.  If you do not show up for your scheduled appointment or cancel with less than a 24 hour notice you may be charged a $25 fee. 

Appointments for additional children should be made by phone before coming to the office.  If you would like another child to be seen, please schedule appointments for both children by phone.

Well and sick child areas are designated.  Please proceed to the sick waiting area if your child has a possibly contagious illness.  Please do not converse on your cell phone in the office and examination rooms.

After Hours Call Service:

After hours calls are answered by B&B Answering Service and Registered Nurses.  The physician will be notified of any emergencies.  We ask that you limit these calls to urgent issues and emergencies only. You may also click on the KidDoc Symptom Checker for sound advice for your child’s care from the American Academy of Pediatrics.  For refills, appointment requests and other non-urgent matters, please call during regular office hours.

Billing:

Our billing is done out of our Parent Office, “Ancor Health Center”. For billing questions, please call (903) 236-8600 ext. 132. We are contracted with most major insurances. Please contact your insurance provider or you may call our office to find out if we accept your insurance plan. All patients are expected to provide us with your current insurance information. Payment is always due at the time services are rendered. We accept cash, checks, debit and credit.

Medications & Refills:

Please allow 48 hours for all prescription refills. For new prescriptions or changes in dosage of ADHD prescriptions, the patient must be seen prior to prescriptions being sent.

Immunizations:

Our practices believe that all children should receive the recommended vaccines according to the guidelines provided by the CDC and AAP. We are happy to discuss your concerns about vaccinations and we are willing to work with parents to ensure we are taking the very best care of your child.

Medical Records:

Our office has 30 business days to release your medical records. There will be $25 charge for copying your child’s chart for the first 30 pages. There may be an additional charge for larger charts. Medical records can be transferred to another physician’s office at no charge.
